Note: You can also make them in the LEDX skin transilluminator in the safe house (level 3 care unit).

My advice

They are both quite rare and difficult to find.
If you've found one in a raid, ESPECIALLY, save it for the quest.
If you want to upgrade your hideout, wait for a second one unless you absolutely must have level 3 care units.
Finding Ophthalmoscopes and LEDX often requires keys and luck.
The map where you can find it most easily = on Littoral.
